As a nonprofit, regional health network providing health care services at 15 campuses and more than 350 sites in Lehigh, Northampton, Carbon, Schuylkill, Bucks, Montgomery, Berks, Monroe and Luzerne counties in Pennsylvania and Warren and Hunterdon counties in New Jersey, St. Luke’s University Health Network (SLUHN) is currently conducting a community health needs assessment (CHNA) as required by the Patient Protection and Affordable Care Act. Residents are being invited to participate in the survey.
Under the Affordable Care Act, all nonprofit hospitals are required to conduct a CHNA every three years. As part of the process, answers to the anonymous survey will be used to help hospital and other officials better understand current health needs within the community.
Anyone who is 18 years or older may take the survey one time. Click here to access and begin taking the assessment.
